The clinicians and staff of Blessed Family Care & Mental Wellness take your confidentiality seriously and complies with the Health Insurance Portability and Accountability Act. The privacy of our patients is respected and, except in a few cases mandated by state and federal law, information about treatment and care will not be released without the patient’s expressed consent.

Booking an appointment

You can schedule an appointment with us by clicking "BOOK NOW" on our website. Please note the the process of booking an appointment includes screening and intake forms. To ensure a smooth stress free process on the day of your appointment, it is encouraged to complete all intake/consent forms at least 72 hrs before your scheduled appointment.

Telehealth

We offer telemedicine services to allow virtual consultations with our patients. Telemedicine is offered via Zoom link that can be used with all of your visits via cell phone or home desktop. When booking an appointment, kindly select "ONLINE" visit for a televisit.

Insurance Plans

At our practice, we accept most major health insurance plans, including Medicare, Medicaid, and commercial insurance. Out-of-pocket pay is also accepted. We understand the importance of providing quality healthcare that is affordable and accessible for all of our patients. We strive to make sure that our services are covered by your insurance provider and are happy to assist you with any coverage questions. Our staff will work with your insurance provider to verify your coverage and explain any co-payments or deductibles you may be responsible for. Please bring your insurance card to your initial visit so that we can accurately assess your coverage.

Payment

While we obtain card information during the intake forms process to confirm the booking, the actual amount due (copay, etc ) is only due at the time of appointment. Your card will not be charged for any reason before your scheduled appointment. We accept cash and credit card. A receipt will be provided that can be submitted to your health care plan for out-of-network benefit reimbursement.

Medications

Regular follow-ups with your routine office visits are important to ensure the quality of the prescriptions to be provided.
Before your clinician/prescriber writes any prescription for you, you must see them first immediately if you are a new patient, if you have missed 2 or more scheduled office visits or the time since your last visit exceeded 90 days.
If you need a refill for your prescription, please inform your clinician/prescriber at least 3 business days before your medication runs out. To be safe, we highly suggest that you set aside an emergency reserve of 3-5 days’ worth of each prescription.

Cancellation Policy

Because your appointment time has been reserved for you, you will be charged for cancellations with less than 48 hours’ (two business days) notice, $100 for new patients and $50 for established patients.
 For example, if your appointment is scheduled on a Monday or following a long weekend, please notify us on the preceding Friday if you will not be able to make the appointment.
 Charges for missed appointments are not covered by your insurance and are due and payable prior to any further appointments.
 In the case that you are more than 15 minutes late and/or do not show up to your scheduled appointment, you are subject to the full no-show fee of $50.

Masks for all staff and patients is optional.
Disposal equipment will be utilized as much as possible
Patient appointments are staggered to assure proper sanitization.
 Optional telehealth services to maximize the comfort level for patients.
